Our Medical Pediatric Clinic puts our pediatric services on wheels and extends access to our patients with transportation or other access issues. The pediatric clinic has the latest in medical equipment and technical support. Walk-ins are welcome; appointments are encouraged. Appointments can be scheduled through our Patient Service Center at 360-377-3776.

Extended Hours Clinics are available at the Clare Avenue Medical Clinic and the Sixth Street Medical Clinic. Both are available to all established PCHS patients. Patients can receive care for minor illnesses during after-business hours. Saturday hours are available at the Clare Avenue Medical Clinic. The Extended Hours Clinics are closed for major holidays.

Mobile Peninsula Ambulatory Team

Mission

The Peninsula Ambulatory Team seeks to help those who need more support around the Peninsula. They work together with you to optimize your mental and physical health.

The Team

This multi-disciplinary team consists of a Community Health Worker, Mental Health Counselor, Substance Use Disorder Professional, and a Primary Care Provider. In collaboration with the Bremerton Fire Department and other community partners, PAT offers proactive services tailored to one’s individual needs.

Purpose

The Peninsula Ambulatory Team works with patients to develop a plan for more sustainable long-term solutions. PAT can help bridge you to that comprehensive care from the comfort of your home.

Mobile PAT Schedule

Services provided Monday – Friday during normal business hours. Specific time decided on case by case basis.

Mobile Dental Clinic

Our Medical Dental Clinic puts our traditional services on wheels to extend services to our patients with transportation or other access issues. The Mobile Dental clinic has top-of-the-line dental equipment (including Xray technology and electronic records) and is wheelchair accessible.

Appointments are encouraged and we help walk-ins whenever possible.
