Mission
The Bremerton Ambulatory Team seeks to help those who need more support. They work together with you to optimize your mental and physical health.
The Team
This multi-disciplinary team consists of a Community Health Worker, Mental Health Counselor, Substance Use Disorder Professional, and a Primary Care Provider. In collaboration with the Bremerton Fire Department and other community partners, BAT offers proactive services tailored to one’s individual needs.
Purpose
The Bremerton Ambulatory Team works with patients to develop a plan for more sustainable long-term solutions. BAT can help bridge you to that comprehensive care from the comfort of your home.
Mobile BAT Schedule
Services provided Monday – Friday during normal business hours. Specific time decided on case by case basis.
